    Important Vaccination Update

    If you or your child were vaccinated at one of our Care Centers between January and June of this year, 2010, you may need to be revaccinated. We have discovered that the some vaccines may not have been stored at the correct temperature. This means that the vaccines may NOT be effective in protecting you or your child.

    How do I know if I have been affected?

    You have been affected if you or your child received a vaccine between January and June this year at one of the following Care Centers:

    San Francisco
    595 Buckingham Way
    1375 Sutter
    Cesar Chavez Primary Care

    Marin County
    Novato Primary Care

    Lake County
    5150 Hill Rd
    Kelseyville

    Sonoma County
    5300 Snyder Lane Family Practice
    Farmer’s Lane Pediatrics
    Landmark Family Practice Suite 202
    Landmark Obstetrics Suite 165
    Landmark Pediatrics Suite 120
    Rohnert Park Pediatrics
    Sebastopol Family Practice 652 Petaluma
    Summerfield Family Practice

    If you received your vaccine at a different Care Center – one that is not listed here, you are NOT impacted.

    What should I do if I (or my child) received a vaccine at one of the above Care Centers between January – June 2010?

    Your health is our number one priority. We strongly recommend that you be re-vaccinated. There is no charge for the visit or vaccine. Please call our toll-free number to schedule an appointment or speak with a nurse at 1-877-657-8987, between 7 a.m. and 7 p.m. Monday to Friday and 9 a.m. to noon on Saturdays.

    Sonoma County patients: we will hold a special central vaccination clinic at 3883 Airway Drive in Santa Rosa. Hours are 7 a.m. to 7 p.m. Monday to Friday and 9 a.m. to 12 noon on Saturdays. Please call the toll-free number at 1-877-657-8987 to make an appointment.

    Although you DO need to be re-vaccinated, there are no short- or long-term harmful effects from the original vaccine, or from being vaccinated again so soon after the initial immunization.

    Re-vaccination is a precautionary measure to make sure that you are fully protected. If you choose not to be re-vaccinated, you may still be vulnerable to illnesses the vaccine was intended to protect you against. Please accept our apologies for any inconvenience this may cause you. The care we provide to patients is our primary concern and we have put into place additional procedures to ensure this will not happen again.
