Family Health Center

The Family Health Center offers primary medical care for adults, adolescents and children. Family health services include preventive care, sick appointments, physical exams and sports physicals, immunizations, mental health evaluation, and treatment of chronic and infectious diseases. Our medical team also includes specialists with expertise in other areas.

The Family Health Center participates in California Pacific Medicl Center's resident teaching program. As such, your health care team includes our board-certified expert physicians, nurses, and:

• Resident physicians (doctors participating in a specialized three-year training program)
• Medical students (pediatrics)
• Specially trained nursing staff available to provide medical advice

Medication Refills
Call your pharmacy and ask that they fax a refill request to us at the number below. Allow 48-72 hours for your prescription request to be processed.

Test Results
We will contact you if your test results are abnormal.

Referrals to Specialists
New referrals may require either a phone call with the advice nurse or your doctor, or you may be asked to come in for an appointment. To request a referral, call 415-600-2402 (adults) or 415-600-2403 (children) and leave a message. Please allow 5 working days for non-urgent referrals to be processed.

Forms and Letters
For forms requiring physician input, please complete and sign the sections that pertain to you prior to submitting the form to us. Be sure to let us know where the form or letter should be sent (contact name and address). Some forms may require that you come in for an appointment. Please allow 1 week for the form or letter to be processed by our office. If you have other questions or concerns, our Family Health Center manager will be happy to assist you.
