Family Health Center
Welcome to The Family Health Center, a collaboration between Sutter Pacific Medical Foundation and California Pacific Medical Center (CPMC). We provide primary care for adult patients from a wide variety of backgrounds through a team based approach. We provide preventive care, acute care and management of chronic diseases, like high blood pressure and diabetes. We also offer a variety of specialty services both on site and through Sutter Pacific Medical Foundation’s network of advanced specialty care.
Our team includes doctors, nurses and medical assistants. We speak English, Cantonese, Spanish, Mandarin, Vietnamese, Korean, German and French. Our physicians include faculty as well as resident physicians of the CPMC Internal Medicine ResidencyProgram. The resident physicians are supervised by our experienced faculty physicians and provide direct patient care.

Patients at the Family Health Center benefit from our team approach to care. We are committed to providing you high quality care in a compassionate and respectful environment. Having a team of health care clinicians enables us to offer convenient appointments and means that many of your health care needs can be met at one convenient location.
The Family Health Center participates in California Pacific Medicl Center's resident teaching program. As such, your health care team includes our board-certified expert physicians, nurses, and:
- Resident physicians (doctors participating in a specialized three-year training program)
- Medical students (pediatrics)
- Specially trained nursing staff available to provide medical advice
The following provides information on frequently asked questions.
Monday through Friday we have appointments from 9AM to 4:30PM daily.
On Thursdays we have extended appointment hours until 6:30PM.
Call your pharmacy and ask that they fax a refill request to us at the number below. Allow 48-72 hours for your prescription request to be processed.

Referrals to Specialists
New referrals may require either a phone call with the advice nurse or your doctor, or you may be asked to come in for an appointment. To request a referral, call 415-600-2402 (adults) or 415-600-2403 (children) and leave a message. Please allow 5 working days for non-urgent referrals to be processed.
Forms and Letters
For forms requiring physician input, please complete and sign the sections that pertain to you prior to submitting the form to us. Be sure to let us know where the form or letter should be sent (contact name and address). Some forms may require that you come in for an appointment. Please allow 1 week for the form or letter to be processed by our office. If you have other questions or concerns, our Family Health Center manager will be happy to assist you.
